We accept most insurance plans. You must get a referral for Physical Therapy from your primary care physician in order to
get reimbursed. If you are not submitting an insurance claim, you can come in directly, without a referral.
Payment
is due at the time the services are rendered. Special payment arrangements are available for those who are not covered by
insurance. Please ask about these special arrangements when you make your first appointment.

Policies:
1) Please bring a pair of shorts, a tee-shirt,
and sneakers at the time of the initial evaluation and for therapy. Additional sweats may be worn for comfort.
2)
The initial evaluation is usually followed immediately that day by the first therapy session. Please allow enough time in
your schedule. The first appointment may take up to 1 1/2 hours, and subsequent sessions may take 45 minutes to an hour.
3)
Patients are responsible for full payment when cancelling less than 24 hours before the scheduled appointment.
4)
Please be on time. Patients arriving more than 15 minutes late without calling may have to be rescheduled.
